I am a candidate to be a participant in the following NSNY sponsored or organized event.

I ASSUME ALL RISKS AND HAZARDS INCIDENTAL TO THE CONDUCT OF THE ACTIVITIES AND TRANSPORTATION TO AND FROM THE ACTIVITIES.

I further hereby release, absolve indemnify and agree to hold harmless the Nebraska Society of New York, (NSNY),  the organizers, directors, sponsors, supervisors, the facility and administration and each of them, from any claim, demand, or action arising out of, or in a way related to the event.

In the event of an injury, the staff is authorized to obtain any medical care or treatment deemed necessary.

I HAVE CAREFULLY READ THE ABOVE WAIVER AND RELEASE AND FULLY UNDERSTAND THIS IS A RELEASE OF LIABILITY AND I AGREE TO IT VOLUNTARY.
